Firstly, break down the form into progress sections. Such that only one section shows per time until it is completed. This makes more mobile friendly and less cumbersome to the user.
Error State: To better handle error states, let each field be validated as soon as it is completed. This would help handle errors without having to wait till the end of the form before the errors are flagged.
Aesthetics: The current form doesn't look appealing, hence, the user would quickly feel bored and worn. Having a beautifully designed form makes is less boring to fill. It also increases the users interaction with the platform.
