What kind of product is this a commercial product or something required for medical patients? Can we make it a little more exciting? Forms should always prioritize functionality but the color schemes and images here are making the experience a little less fun than it could be.
Can we design for a mobile first scenario? I find that designing for mobile first - since these devices are virtually ubiquitous now - gives a lot more clarity when translating designs to desktop views.
Let's break up the 3 sections into 3 different screens. And to be respectful of the user and their time, let's provide a progress indicator with either "Step x of 3" or a percentage. I don't like the 2 columns of information that jump down to 1. It makes it look like there is even more to fill out than there actually is and has the users visual cues jumping all over. Let's streamline this. Also, we can use floating labels on the fields so we can save space.
What does the backend look like? Can we give the user asynchronous feedback for the form validations? Immediate, real-time feedback on the front end is always most convenient for users so that they are not surprised with a big scary notification after they spent all kinds of time carefully completing a form. I'd like to validate mission critical fields right away so there are no surprises.
It would be great to detect prescription medication names and suggest them automatically as users type. Potentially a lot of data to deal with, but it should help to eliminate errors and allow us to collect a better data set overall. It's important that this is easy for users. I created a short animation of my idea for this feature and I can move forward with more specs if you like it. What do you think? Fonts for this mockup are tentative, we can adjust them as we see fit.
