Compatibility with prerender v5 #13

Hi! Thanks for this plugin I've been using it extensively.

Prerender v5 just went out and it replaced PhantomJS by Headless Chrome. The plugins API has changed and beforePhantomRequest and afterPhantomRequest have been replaced respectively by pageLoaded and the content is in req.prerender.content.

Could you check that out?

Actually I just noticed #12 was already opened so work in progress.
