D'oh, the shutdown pin for the DAC is tied to ground, but this permanently shuts down the device.
As a kludge, carefully bend pin 6 of the DAC up, and tie it to 3.3v somehow.
Please eventually respin the main pcb and fix this issue by tying pin 6 of the MCP4821 DAC to logical high instead of low.
Consider changing the footprint to SOIC8 when you do this, there is no pressing need for the smaller MSOP footprint.
