As far as I know, IPS files cannot change the filename, you'll have to do that yourself. As for retropatcher, it looks like there might be a bug with large ROMs like this, I'll talk to Jonny and see what we can do to fix it.
As for the audio, I suspect it's an Analogue Pocket Hardware issue, there are a few audio issues still with the core and this does some odd things. In any case, I can take a look at it again tonight, but I suspect that it's a problem with the core and not something I can fix. Are you able to test with a flash card?
I tested with a flash card and can confirm that it's not a .pocket bug, it sounds the same as with the patch. I'll mark this as an Analogue hardware bug. Hopefully it'll be fixed with the next patch, there are definitely some known audio issues.
When using using either retropatcher or lunarips to apply the .pocket patch, neither outputs a .pocket file, rather the file stays .gbc. After manually changing the file extension and playing on the AP, video appears good but audio is highly corrupted.
