Feature: Support remote Ollama/GPT4All installation #199

Hello and thanks for this great repository.

It would be nice to have a field at the CodeGPT settings, to input the URL / IP number and port number of Ollama / GPT4All installation.

None of these options work: Extension.js or custom. Use the extension "Continue" instead of this junk. That one works easily with a remotelly installed Ollama.
