Update paper and bibtex #96

Thank you @jbytecode for your time in these amendements. I am willing to merge this but regarding the third change, I actually marginally prefer the original layout where the table wasn't split over two pages (and where the logical flow may be viewed to be more natural).

I understand that the Statement of Need is a critical section; likely more critical than Design Principles. Would it resonate with you if we reorder only these two? This is actually the form (Design Principles at the end) that the paper took early on before the review.
