List of changes:

Generated completely new input data for column generation and exploiting sparsity. Previous input data for these two problems were from OPL examples.
Reference added for the input data for Benders decomposition, which is from page 139, Integer Programming by Garfinkel and Nemhauser, 1972.
Fixed some typos.
