make Consent and Waiver of Hearing ready #195

@tuckersamuelsen I'm trying to determine if this form is complete and ready for use. Can you confirm that the prosecutor's information at the top doesn't need to be filled out using this form (i.e. it would be written in manually)? Also, following the words, "The undersigned has reviewed Petitioner's Petition to Expunge Record....", would the prosecutor check the boxes manually? Or does this form need to check the boxes?

Yes, this form is done. The form is primarily filled out by the prosecutor, so none of the additional information needs to be filled out by the user. The prosecutor will check the boxes and put their information at the top.
