first pass on reminder to print two copies #199

This addresses item 11 of Form Bank Issues #183. Created alert to trigger reminder to print two copies on Petition forms only.

@benschinn and I paired on this. (Thanks Ben!) This gets things working. Would like some feedback on future iterations to enhance user experience.

@lukeschunk I think it's good to go as a first cut. We may want to do future iterations for better pattern or better UX/UI
