Issue 103: Demo Menu: Add device responsive menu folding #107

Description

For work toward issue #103 ' Demo.html should have nice menu bar - this issue is for first-pass version'.

Replace #104
Add device responsive menu folding.
- Menu will be viewable/usable on phones and other small devices.
Properly gray out the non-active menu items.
Change to dark style.
Update menu items to closer to 'top bar' items as described in the architecture presentation at: https://docs.google.com/presentation/d/1DPunsJgNrGR4Bk5FJrbbkoKMmRxOkhC6p_GN2BtRUX8/edit#slide=id.g88d7f0fe54_0_138

Improvements to the user interface will help users adopt Wellioviz.
