no "index.html" is created #1

I don't know why, but no "index.html" gets currently created anymore, thus http://kwb-r.github.io/fakin.doc/ displays the document not by default.

However, this works for a similar setting, e.g. here: https://github.com/rstudio/bookdown/tree/master/inst/examples

closed but know "Introduction" chapter starts with "1"
