Get the text and scoring of a few evaluations that may be added to our MHA app

Stress Index from Power Point from the Canadian mental health association added.

Added Patient Health Questionnaire (PHQ-9) Also called Depression Patient Health Questionnaire From https://www2.gov.bc.ca/assets/gov/health/practitioner-pro/bc-guidelines/depression_patient_health_questionnaire.pdf

This is the second common type of score with is by Frequency rather than just Yes / No

Added Patient Health Questionnaire-4, Very Short. more diagnostic, not likely to use

Added the DSM-IV Criteria for Major Depressive Episode.

This is more informational for us than to be used in the app I think.

These three type look pretty good, have a example of Each. Thinking using the first Stress one, the PHQ-9, and the GAD-7 Will open again if find better ones or if feedback shows any to be not good for an app.
