[Feature-Request] Extend Physical Data with more fields

Story As a patient I want to add more information to my physical data e.g. head/arm/… circumference as a permanent (optional) field.

What would be needed?

• A control to add new fields and their unit - maybe as a button on the interface to enter physical data
• A dynamic physical data entry which shows all fields which can be entered
• the table needs to adapt dynamicly - what happens if there are too many fields, possibility to hide fields from the table
