Explanation of the issue:
The Cooperative of Belgium (Syndicalist Belgium formed peacefully) has the default Belgian flag in the upper left corner, but the syndicalist Belgian flag on all its units and in the save files.

Steps to reproduce:
Start as Flanders-Wallonia
Go down the path that results in an independent Belgium
Become a republic in the event
Elect the Jacquemotte wing of the BWP (There is no localization for this option; choose the one that gives syndicalist party popularity)
Go down the Reform the Party branch of the focus tree all the way to the Belgian Cooperative Union focus
Take the Belgian Cooperative Union Focus
Click the only option for the event that follows and then you're done

The proper syndicalist Belgian flag is used when syndicalist Wallonia forms Belgium after controlling Flanders. Therefore, the only way this issue is caused is when the Cooperative of Belgium forms peacefully.
Possible cause:
I'd imagine that the Cooperative of Belgium focus assigns the wrong cosmetic tag to Belgium or the wrong flag is used in the cosmetic tag that it is assigned.
