Explanation of the issue:
Doing "Proclaim the Republic" focus makes your Head of State Dr. Malan and renames you to "Republic of South Africa", the problem is that doing it before Trek reenactments ends messes up with "The Centenary Celebration Arrives in Pretoria" event's effects, as they end up renaming you to Republic of South Africa that you already are and while its supposed to change Head of State to Dr. Malan it instead changes it to Paternal Autocrat generic.
Steps to reproduce:
Elect National Party in 1938 elections
Proclaim the Republic before the trek reenactments event chain ends
