Added setup.sql #4

I thought it would be good to add a setup.sql file, so the people who aren't very advanced with SQL have an easy way to set up their database.

@SkilzMastr I think this is a great idea!

It would be helpful to keep a clean gitlog if you squashed or at least soft reset and turned this into one commit. More info here on this StackOverflow post
