Sometimes a user will provide consent to some purposes but not to others, and it is valuable to record that consent was not given.

For each purpose in the receipt, an additional flag should indicate whether consent was given or not.
Describe alternatives you've considered
Additional context
This would make it possible to unambiguously revoke consent that was granted in a previous interaction, so relates to #13
