At the end of each path, need to make sure text field captures email address for sending @smsliman @KarolinaGroszewska

@smsliman with the information from the meeting now being available, we only need to collect the email if they select they would like the paperwork sent over? does that mean creating a second button for the instance of sending it to SAC without an email (where the option with email displays the legalese)?
