Various issues plus patch #29

Patch against r393. Fixes a few issues:
- Failure with v1.0 crash reports that do not include machine_info ("Could not 
decode crash log: Crash report is missing Machine Information section")
- Truncated output of "plcrashutil convert" (misplaced NULL character which 
isn't necessary anyway).
- OS build version still prints as "(TODO)".
