I have "Show only 1 non-modifier key at a time" enabled.
If I press Shift, release it, and press another key while the Shift key is still displayed, Keystro will show both Shift and the other key at the same time, even though Shift was not held while the other key was pressed. This applies to all modifier keys (command, alt/option, ctrl).
I would expect the display of modifier keys to be cleared when another key is pressed without those modifiers being held. This might belong as part of the "Show only 1 non-modifier key at a time" setting, or it might belong under a new setting, since I expect some users might want to just show the sequence of keys pressed without regard for whether they are modifiers or not.
