Claim Network Form

As a user, I want to be able to claim a network by filling out a form on the website so that I can register the network under my account.

Acceptance Criteria

• [ ] User can access claim form after authentication.
• [ ] Form includes fields for network name, address, and description.
• [ ] Form validates inputs for correct format before submission.
• [ ] User receives confirmation message upon successful submission.
• [ ] Claim request is stored in the database for review.
• [ ] User cannot claim a network already claimed by another user.
• [ ] User can view status of claim request after submission.
