Add bug-reporting link to UI #46

There isn't an obvious link to this issue tracker to report site problems. Probably adding this to the site nav, maybe even with a separate "Report Site Issue" button in the topbar, would be most useful. We may want to add some templates in the tracker too - actual site UI issues vs. problem content in one of the components, for example. We should kick this around a bit before settling on a full solution but just a link to the tracker here is straightforward.

Suggestion: add a "Feedback" pulldown menu which links to the new issue UI on this repository. Adding issue templates can come later.
