Issue importing data from Samaritan #22

Having issues importing. Seems to have successfully connected to the SQL Server, but I have the following error:

Transform Data, "System.IndexOutOfRangeException: Index was outside the bounds of the array. at Bulldozer.CSV.CSVComponent.LoadIndividuals(CSVInstance csvData) at Bulldozer.CSV.CSVComponent.TransformData(Dictionary 2 settings) at Bulldozer.ProgressPage.bwImportData_DoWork(Object sender, DoWorkEventArgs e)

Was hoping someone could help me get through this. Thanks so much!
