In order to ensure/improve the quality of our postmortems, consider integrating postmortem reviews in our process, similarly to what is described in PagerDuty's original docs.
Such a review process needs to be owned by the Production forum.
We can have a monthly postmortem review meeting to go over recent postmortems, and refer to the following points:
Was the postmortem completed in a timely manner (within a week from the incident)?
Was the description & timeline complete and clear? Can we fully understand what happened from the postmortem summary?
Root cause analysis quality - did the postmortem uncover the actual root cause(s)? Do we need to ask the postmortem owner to follow up with a deeper analysis?
Action items:
Are there action items?
Are the action items clear and well-defined?
Is there an assigned owner for each action item?
Is there a Jira ticket for each action item (with a postmortem-action-item label)?
