First spiral layer should ramp up extrusion rate #48

The first spiral layer uses the same extrison rate throughout but should ramp up from the first flat layer to the end of the first spiral layer by ramping up the extrusion rate form 0 to the final layer - a version of the gcode function could do this.

First layer is flat, rampling only starts with second layer
