3D view blank when loading image after CTA Head and Neck sample #446

Reproduce:

  1. Load CTA Head and Neck sample.
  2. Load MRI Prosatex sample.
  3. Load CTA Head and Neck again.
  4. Load MRI Prosatex sample again.
  5. Bug: 3D view blank (until changing camera position)

This also looks like a bug with setting the proper transfer functions on the volume.
