Update create dataset from database modal layout #456

The layout has been updated for aesthetic reasons. The functionality remains the same. A small code bug has been fixed.

This PR addresses issue https://github.com/Kitware/minerva/issues/424

New layout: chrome_2017-11-01_16-21-08

Existing layout: chrome_2017-11-01_16-22-09
