Change "Exam" back to "Study", "CMRN to "Patient ID" #104

Please change Exam back to Study as this is a Mayo thing. The proper DICOM term for the public is Study. Also for the public, change CMRN back to Patient ID. This is more useful to the public and are Mayo proprietary terms.

This public version should be something more conducive to public usage.
