Update ehr and image browser UI

This is a follow up to #16 for updating the UI. We can pin this down more precisely when we get to it, but here are some initial thoughts:

• Rename DICOMWeb tab to something like "patient images". And reorder tabs so that EHR comes first, then images, then remote.

• Patient images (DICOMWeb) tab should not show the local dicomweb server in the list. It also doesn't need to show the top level patient as an expandable object, since there will always be at most one patient selected at a time.

• Patient records should be scrollable with a nice display that can handle lots of patients. For example pagination and/or a search filter based on patient name or something.

As part of this let's also update the documentation:

• explain in readme how the link between EHR and the dicom server works. e.g. explain VITE_PATIENT_IDENTIFIER.

• Cannot re-click image after loading once.
• Image List not scrollable
• Update ReadMe describing VITE_PATIENT_IDENTIFIER.
  • Add details in "Local FHIR server using data from the LungAIR research group"
