Add UI elements for connecting to a local fhir server, as an alternative to Cerner test patient login
Establish connection to local fhir server and for now just dump a list of patients

For now this can be tested using the random data option of fhir-sandbox

~~As part of this issue~~In a later issue we should include not only dumping a list of patients but also the ability to select a patient and have the currently selected patient be a variable that updates in the global store.

The "currently selected patient" concept will be needed for #16
