File reader changes #867

The file reader is now a bit less platform-specific and allows to hide platform complexity.

In the long term I believe this can be used to:

remove kinc_image_size_from_encoded_bytes, kinc_image_init_from_callbacks etc. or at least greatly simplify the implementation
implement compressed file reading
allow the user to plug in their own filesystem implementation
some other simplifications in NDA platforms that we can discuss in private
