That's a pretty old video though. For a more recent take on the subject, MCC did a video featuring a similar system to be released in their next update.

And in all honesty, i think these kind of features would be way more suited in AGM than MCC.
MCC seems to implement some features that Dslyecxis shows, but actually less than his. So I'm not sure how the older video is less relevant.
That said, AGM alread does everything that this MCC video shows, it just doesn't let a grenade float in front of your face. Which is an awkward indicator for your throwing mode anyways, but that's just my opinion.

That was included at some point, but it was removed; due to arma physics the grenades didn't roll reliably; sometimes they got stuck on the ground near you with bad consequences.

All in all, i think that what the grenade system is truely missing is a way to see where you grenade will go. Dslyecxis did it by putting the model in front of your face, and MCC is doing it in a similar fashion with a 2D picture if i'm not mistaken. If i'm wrong and it's not the case, putting a grenade in the middle of your screen just to indicate the throwing mode is indeed a bit awkward.
I can't stand MCC doing what's doing now, great features and all... but not modular or anything. Mission generating is great but I won't use it due to everything else that duplicates or does worse than other mods that are much more mandatory. Sorry, had to comment. :P
Back on topic, grenade cook-offs would be great, but I believe it was already discussed in another issue and closed with no suitable animations for holding the grenade up or something similar.
On rolling... bad geometry on everything is very bad for Arma, many things you can't do reliably because of it (also why I think BIS holds away from adding realistic bipods... their map geometry is simply not good enough and it would be buggy as well, like deploying bipod 2m above a rock or something).
Indication could simply be made with a small arrow or something, or a small circle of where the grenade will be thrown from (from I say, basically replace the grenade as in Dslyecxi's video with a circle). For starters I think it would work great and it would allow to get other functionality in and see if it works well enough before making it nice.

I'm pretty sure that is the case. It always is centered on the screen (on Dslyecxi's video it's a bit lower, but it stays there in all modes so it's no poor mans CCIP either; it doesn't block your view as much though). I mean its rotation shows the mode but I don't really see the point in that either - why would you hold a grenade in a 45° angle to throw it straight instead of in an arc? Text does the trick just as well.
I like Dslyecxi's version up there. Seems very useable.
possibility reworking of grenades ad ability to cook of better positioning eg easer to throw through windows over walls mabey even throw back
