In PvP the two pain effects present very differently. The newer "alternative" pain effect is very advantageous due to the lack of blurred vision. If not all players remember to manually switch to "alternative" pain effects, some will have a disadvantage while playing.
Suggested solution:
Add a new function to the existing 2D editor module "Medical System" whereas the mission maker can force a specific pain effect onto all players, thus levelling the plain field.
This would be great as my group would love to turn the alternative pain effects off entirely but others might feel the other way as @JamesTheClarke shows here.
