Numerous missions prediction failing #49

New missions which are giving experience tokes are constantly failing even with predictions of 50% or more HP left! I have the latest version "updated" for 9.1.0

I am having numerous missions fail across multiple characters. It has only started happening since the first of the year for me. The prediction is always a green 'win', with no partial percentages.

It's happening more and more frequently. It happens on the new missions for 9.2 mats and also old one. It happens on characters that I've maxed and are doing ZM and also on alts that are still in 9.0 content. In the posted link, a mission shows 'complete' but there is no reward and it doesn't show up in the chat log or my bags.
