Add a URL next to the email field for the comments

I need on the blog part of this website to insert and URL next to the email 
field.

For example on this page:
http://www.afarcasa.com/publicita-gratis/ where the user leaves a comment I 
need the following URL: Informativa Privacy that should take the user to 
http://www.afarcasa.com/informativa-privacy/

I played a lot with the coding of this theme since I manually inserted menus, 
logo, share buttons and removed some parts. But I am unable to find where 
should I go to add this link next to the email field. so the form will look 
like this:

Nome

Email Informativa privacy

Sito web

This link is a must here in Italy.
Great theme and especially the last version is great.
Hope you can help me. Thank you.
