Only record a line as dead after two consecutive attempts to contact it have failed #61

Due to various problems with mobile systems it is possible for a line to mistakenly identified as dead. This change means a line will only be marked as dead after interviewers have twice (consecutively) marked it as dead.

It will initially be recorded as a no reply
