CATI "Productivty" indicator for interviewers #82

Add the following to pause screen for interviewers

  1. Initial time of first log on of the day
  2. Number of interviews done
  3. Productivity calculated using the formula (3.8 x (income earned - interviewer cost)) (rounded to the nearest integer). Two figures are needed - one for today - the other for month-to-date (starting from the first of the month).
  4. A ranking (e.g. decile) for the above 2 productivity figures, so that each interviewer knows how they compare versus the rest of the team.

This change also improves the performance of CATI Active schedules and the CATI Summary Report for interviewers.

Added a change to the way Shift Allowance is calculated. What happens now is that interviewers are given 5 minutes per hour but only if they take it. i.e. they can accumulate a shift allowance up to 5 minutes but no more.

They only receive the shift allowance when the system is paused and they either start work again or log out.

The reports are updated to show these new shift allowances if the reporting period starts on or after Monday 27th July 2015
