Adds Issue Template - related #157 #179

As this template asks for a product version, a stable release process should be used (see #157) to identify which version of the codebase is used. This should close #138, close #140, close #141, close #143, close #146, close #150, close #151, close #152, close #153, close #154, close #155, close #156, close #158, close #159, close #160, close #161, close #164 and close #168

@KyranRana Thank you. I'd like to help you more to unstuck people but the last points remaining on #157 rely on you. Only you are the authorized user to create tags and install packagist webhook that could avoid false positive issues. As I stated in some of your issues, I made a automatic fork that accomplish this work but it's done without any logic and produce wrong tags.
