Update recommendation docs to reflect latest decisions #113

@jak473 @sfolsom @melanieWacker This is a big job, but I fear that the modeling recommendations will be confusing if they are not in sync with ontology specifications.

I don't mind volunteering to take a lead but it's questionable what I can achieve by June 30.
