Support response header for all authorities #466

This provides a consistent API that is easier for clients to consume.

For example, https://lookup.ld4l.org/authorities/search/local/publisher_cities_select_list?q=los+angeles&maxRecords=8&lang=en&context=true&response_header=true&startRecord=1 does not return a response headerr.
