so that image is usable outside of local dev and CI.

hopefully this... fixes #96

i pushed the updated ld4p/trellis-ext-db docker image that the changes in this PR would produce, so that @rsmith11 can test with it.

if that goes well, and if there are no objections to the approach here, we can go ahead and merge.

this allows us to specify a DB connection in the same way as the vanilla trellis-ext-db image, but with our other config customizations (e.g. for JWT authN). see vanilla docs here: https://github.com/trellis-ldp/trellis/wiki/Dockerized-Trellis
