Broken internal links to index pages for schemas after 4.4.0 #11

The readme links to index pages for the 4.4.0 and 4.5.0-draft schemas that are not found (404).

I think ideally the readme would link to the asciidoc source and the gh-pages would link to the rendered html, but that's probably over-achieving.
