Not yet. At present we only support Observation-based extraction. For updating the patient resource, you would want definition-based extraction (http://hl7.org/fhir/uv/sdc/extraction.html#definition-extract), but we haven't implemented that yet. For now, you would need to ask LHC-Forms for the QuestionnaireResponse, and map the data to a Patient resource in your application.
