Feedback form for the sprint week

Goals and deliverable

The goal here is to come up with and finalize the questions to ask in the feedback form for the sprint week. While the questions will be finalized by the Meetings Committee and the Logistics Organizing Committee for the Sprint Week, anyone interested in contributing to the effort is welcome.

We'll work in this google doc. The reach goal would be to come up with all the questions. The stretch goal would be to finalize the wording of each question and be ready to move things over to a google form.

okay we'll be meeting at 10.30am PT in the wonder.me room. In the unlikely event of more than 15 people attending the meeting, we'll use DESC #20.

@plazas @sukhdeep1989 and i just met and went through the google doc linked above; i had adapted the questions from the July form for the Sprint week beforehand. there are various questions that we think can be deleted; we've highlighted them alongside other suggestions.

the draft questions are now shared with the Meetings Committee, the LOC, and the MAC for feedback.
