Template feedback forms for the Meetings Committee

Goals and deliverable

The goal here is to create templates for (short) feedback forms for use by the Meetings Committee. These would include the form for the Meeting Contact Persons (MCPs), Session Facilitators/Chairs, and the LOC/SOC. While the questions will be finalized by the Meetings Committee, anyone interested in contributing to the effort is welcome.

We'll work in this google doc. The reach goal would be to come up with all the questions. The stretch goal would be to finalize the wording of each question and be ready to move things over to a google form.

• Asking clear, concise, and possibly tough questions;
• Contributing to the betterment of DESC events

Detailed description

We would like to establish template forms that can be used for each collaboration event. Specifically, we would like

• A feedback form for the Meeting Contact Persons (MCPs). We will use the form we sent out after the July meeting as a starting point for the template.
• A feedback form for the Session Facilitators/Chairs. We will use the questions we sent out after the July meeting as a starting point for the template.
• A feedback form for the LOC/SOC members to provide feedback to the Meetings Committee re their experience.

i just added template drafts in the google doc linked above. @plazas @yymao can you please take a look? i'm happy to meet tomorrow or friday if you want to go over the questions together. once i have some initial comments, i can share the templates with the meetings committee etc.
