added procedures category and cormobidity to features validation #42

  1. opertn_count, comorb_count and comorb_cat added to validation pipeline
  2. Comorbidity feature added to build pipeline for APC
  3. 'Yes', 'No' and 'Missing' categories added for procedures build pipeline for APC
