[mod_mam_sql] update to support the latest version (0.3) of XEP 0313 #74

Currently the mod_mam_sql module complies with an old version of XEP 0313. 
mod_mam has been already updated to follow up the new version of XEP 0313, but 
mod_mam_sql still hasn't.
