Include FormAccessible trait in Form helper documentation and bring them up to date with Laravel 5.2 #29

As per https://laracasts.com/discuss/channels/laravel/form-model-accessors-doesnt-work, the FormAccessible trait needs to be included, otherwise the code shown in the documentation will not work.

I've also made some updates to the code samples to update them for Laravel 5.2

This should be added ASAP. I spent hours trying to figure out why nothing I was doing was working, even though I was following the documentation.

Does anybody even look at PR in these repos? 1 month and no reaction.

Hey @adamgoose need some help with maintaining this repo? I stumbled upon this issue, searched my code for an hour, found the aforementioned issue and then this PR. The docs could definitely use some love. :) I realize it’s a lot of work so this is not a critique just a proposition to contribute if you need some.
