Standardize the project folder/file structure

Standard:

• .gitignore
• .npmignore
• LICENSE
• README.md
• bin - holds an executable for the modules
• doc
• examples
• lib - holds the code for the module
• man
• package.json
• src - Holds Coffeescript or source files
• test

Changes:

• demo -> examples
• doc -> docs
• LICENSE.txt -> LICENSE
• readme.md -> README.md
• dev\tests -> test
• dev\metrics -> test
• For package.json, test for a test command. "scripts":{"test": "node ./test/js/Ratio-testcases.js"}

Version 0.3.4 is good except that the folder doc should be docs.

The ratio script should be in the lib folder, not src. src is used for other non-script files.
